Patient and Family Experience Surveys

Dear Patient/Family member,

On behalf of CHEO’s Board of Trustees, I am asking for your assistance in evaluating the care and services that you received during your recent visit to our hospital. Your experience is very important to us. At CHEO we strive to provide the best possible care. Feedback from youth and families is key to finding out what we are doing right and where we can make improvements.

  • Survey about your experience with one of CHEO’s clinics, medical imaging department or daycare surgery
  • Patient and families who have come to the Emergency Department, or were admitted to one of the units, and who provide their e-mail address to the admitting clerk will be e-mailed a survey about their experiences of care

As we receive feedback, we continuously use it to make changes at CHEO. Therefore, even if you have filled out a CHEO survey before, please take the time to fill it out again so we can see if we’re improving over time. As with our approach in general, we ask that the child or youth who received care at CHEO fill in the survey as much as possible.

The surveys are anonymous unless you choose to include your name. We also welcome written comments in the section at the end of the surveys. If you have any questions or comments about the survey please call our Manager for Patient Experience at (613) 737-7600 x 2203.
