For the following types of patients:
- Growth and/or puberty disorders
- Thyroid disorders
- Hypoglycaemia of suspected endocrine origin (not metabolics)
- Other known or suspected endocrine problems of childhood or adolescence
| For the following types of patients:
- Bone health
- Obesity
- Lipids
- Known or suspected diabetes
- Dysmenorrhea or dysfunctional uterine bleeding (DUB)

Making a Referral
Mail or fax the referral form to:

- NOTE: Referrals for ambiguous genitalia in children less than 1 month of age OR hyperthyroidism should be called to the Endocrine Doctor-on-call
- Please include with your referral:
- growth chart with all known heights and weights plotted
- relevant lab data
- X-ray reports
- original film for bone age if done outside of CHEO.
- Please indicate on the referral if an interpreter is required and for which language if not English or French.
- Once the referral has been received, reviewed and triaged, an appointment will be booked.
- The patient will be notified directly with their appointment time.
- If the status of your patient changes, it is your responsibility to notify CHEO.
Provide This Information for Patients and Families
- The Clinic is located in C-1.
- Bring the following with you to your appointment:
- health card
- any medications
- original film for bone age if done outside of CHEO.
- A medical student, pediatric, or endocrine resident will usually see the patients first. Height, weight, and pubertal examinations are performed on all patients. X-rays and blood tests may be performed that day or booked for a subsequent day.
