On September 7, 2021, the Honourable Christine Elliott, Deputy Premier and Minister of Health, issued an order under the Connecting Care Act, 2019, transferring local pediatric home care services and staff from Home and Community Care Support Services Champlain to CHEO, effective September 20, 2021.

This transfer will enable an innovative local approach to integrate pediatric home care with other health care services and is part of the province’s plan to build a connected health care system centred around the need of patients.
Ensuring continuity of care is a top priority. The process will be as seamless as possible for families, service providers and staff. Patients and families will continue to access home care services in the same way and through the same contacts they know and trust.
